Medera Brain and Spine Centre, we would like to state that you have rights specified in Article 11 of the Law No. 6698 on the Protection of Personal Data (“Law”) and granted to data subjects. In accordance with Article 13 of the Law, our Clinic, which is the data controller, has published the Clarification Text on the Protection of Personal Data on its website http://mederabeyinveomurgamerkezi.com/ regarding the conditions of processing personal data, data security and destruction procedures and principles.

As a data subject, you can send us your requests regarding personal data within the scope of Article 13 of the Law and Article 5 of the Communiqué on the Procedures and Principles of Application to the Data Controller by using this Data Subject Application Form (“Application Form”).

DATA SUBJECT RELATED PERSON RIGHTS

As a data owner, the rights granted to you by the Law are as follows, and you can submit your requests to us in written form and in Turkish by using the application method shown in this Application Form.

You can make the following requests with the Application Form:

To learn whether your personal data are processed or not,
To request information if your personal data has been processed,
To learn the purpose of processing your personal data and whether they are used in accordance with their purpose,
To know the third parties to whom personal data are transferred domestically or abroad,
To request correction of personal data in case of incomplete or incorrect processing,
Processed in accordance with the law and legislation
